LifeWell offers maximum flexibility in plan design. We let our customers (the employer) determine the plan design including features like co-pays, deductibles, eligible charges, primary care physician selection, referral requirements, percentages at which benefits are paid and a host of other options.

Our designs include familiar options like HSA, PPO, POS and self-funded HMO and HRAs.
